Maintaining Your Work Requirements Documentation
The Work Requirements section of your account must be complete and up-to-date in order to submit invoices to Workrise. You will need to update select documents under Work Requirements on an annual basis. We will begin displaying alerts within your account 30 days prior to your documentation expiring, so you never have to worry about falling out of compliance.
To complete your Work Requirements, please have the following documents ready:
- Tax forms: W-9 or W-8BEN-E (for international taxes)
- Bank forms
- Certificate of Insurance
- OSHA forms for the past 3 years
- Your most recent EMR letter
- Safety program documentation
If you’re new to Workrise and haven’t created an account yet, you’ll receive an email from support@workrise.com with the subject line, “You’ve been invited to Workrise”. Click Join Workrise, then create a password to access your account.
If you already created an account, sign in and navigate to the Work Requirements tab.
Work Requirements is divided into two sections, General and Safety. Please complete both sections.
Updating Your General Work Requirements
Under General, please provide all of your tax, payment, and insurance documentation by clicking the Edit button to the right of each section.
Don’t forget to save your changes by using the Save button at the bottom right of an edit page.
Tax Information
Please upload your completed Form W-9 or W-8BEN-E for international companies. These forms can be found at www.irs.gov/forms or obtained from your tax advisor.
Payment Information
Download, complete, and upload the Electronic Funds Transfer Form available through the Payment Information section of Work Requirements page and provide a voided check or a letter from your bank with ACH information.
Upload all of these documents under Bank Forms.
Certificate of Insurance
A certificate of insurance is a document used to provide information on specific insurance coverage. The certificate provides verification of the insurance and usually contains information on types and limits of coverage, insurance company, policy number, entity insured, and the policies' effective periods. Companies must provide an insurance policy that meets the operator’s insurance requirements. Exemptions can be made with approval which is dependent on your company’s line of service.
Please upload a copy of your most recent insurance that names RUSCO Operating, LLC/RUSCO Group as additionally insured and the certificate holder. Additionally, please provide evidence in the COI of additional insured status and waiver of subrogation in accordance with the MSA requirements (also state if you have a blanket waiver of subrogation and blanket additional insured endorsement).

Workrise’s Master Service Agreement (MSA)
The Workrise MSA allows service companies to gain access to Workrise’s network of energy companies.
The Workrise supply chain team manages this workflow via email, and will upload the final signed copy to the Work Requirements page on your behalf once all parties have agreed and countersigned. You will be able to download a copy for your records.
Updating Your Safety Work Requirements
Under Safety, please provide the following documents by clicking the Edit button to the right of each section.
- OSHA Forms
- Experience Modification Rate Letter
- Safety Program documentation
- DOT Questionnaire

OSHA Forms
Providing your safety statistics via signed documents such as OSHA Forms is an important way to show your safety record to new and existing clients. Workrise asks that all companies with more than 10 employees provide their 300 and 300a forms for the past three years for verification.
Complete the short questionnaire and upload your forms accordingly.
Experience Modification Rate (EMR) Letter
The Experience Modification Rate (EMR) is a calculation used by insurance firms to price the cost of workers’ compensation premiums. This rating is used by energy companies to evaluate your company’s safety performance over time.
Please indicate whether your business qualifies for an Experience Modification Rate and upload the letter if so.
Safety Programs
We review your safety programs to ensure you have the right policies in place to work safely on a client’s job site. Both your existing and prospective clients will see this information as they evaluate vendors offering your services through the Workrise network.
Upload a copy of your company’s safety program for review. At a minimum, all companies should have a PDF of their safety program with the following:
- Drug & Alcohol Policy
- Personal Protective Equipment
- Job Safety Analysis
- Short Service Employee
Additional protocols may be required based on your scope of work such as:
- Lockout Tagout
- Hazard Communication
- Driver Safety
You can upload up to 10 PDF documents for review. Submitting proof of implementation such as Training Rosters or Job Safety Analyses are helpful to accelerate the safety review process.
DOT
The Federal Motor Carrier Safety Administration (FMCSA) provides resources on what types of work fall under their remit. To tailor your safety review, please complete the short questionnaire through the DOT section of your Workrise account.
Completing the questionnaire in this section is required regardless of the service types you provide.
Checking Your Document Status
After completing a section, the status next to each section will update to Submitted.
Workrise will check your documents. If revisions are needed, you’ll see an updated status with notes about what revisions are required.
Once your documents have been approved, the status will change to Approved.
Workrise will monitor your document expiration dates and let you know when it’s time to update your Certificate of Insurance, EMR, OSHA forms, or other important compliance documentation.
In order to submit invoices, your safety and insurance documentation within the Work Requirements section of your account must be up-to-date. You can continue to submit invoices for any work completed for the dates all documents were compliant, but once a required document expires, you must update it to continue submitting invoices for work completed after that document’s expiration date.
You’ll know documents need attention when you see a number appear next to your Work Requirements tab or see an Incomplete Work Requirements alert in other sections of your account. You’ll begin receiving these alerts 30 days prior to the expiration date.
